Your questionnaire helps us get to know you before you arrive.
Start with your questionnaire
Please complete it as soon as possible, but at least two business days prior to your appointment so our team can review your information before your visit. (If you are short-scheduled, complete your questionnaire and turn it in right away).
The link below opens our New Patient Questionnaire Access Form. Enter your name and email to create your personal questionnaire. If you would like to save your work and complete it later, you will be able to create an account to do so.

Before you arrive
A few things to take care of
- Complete your questionnaire. Send it at least two days prior to your appointment to allow time for your doctor to review it and get any follow-up questions answered.
- Send recent lab results. Please send any labs from the past year. You can drag them into your Questionnaire’s first page or email them to: https://sendsafe.to/patientservices@ahic.comcastbiz.net. Call Reception if you have questions about sending them.
- Confirm your appointment. If we have been unable to reach you, call, text, or email us. Unconfirmed appointments may be reassigned to patients on our waiting list.
Plan a an hour to 90 minutes
Your first visit includes a consultation and testing session with the doctor. Please arrive a little early; the schedule may not allow a late start.
Please arrive fragrance-free
We see patients with severe allergies. Please avoid perfume, cologne, strong aftershave, and essential oils.

Once you enter the office park
Continue away from Powers Ferry Road. Building 14 is in the center of the park, with numbers on the building and on signposts. We occupy the entire first floor. The clinic entrance faces the back of the park.

If your plans change
Cancel or reschedule at least 48 hours before your visit to receive a full refund of the visit fee charged. Missed appointments are not refundable.
We reserve an hour of the doctor’s time for your visit. Advance notice gives us time to offer that appointment to another patient.
